Elevator Action, made by Brazil company Dismac, seems to me to be a 100% perfect port of the original arcade game. Differences, cartwise, is its solid black cart and new, cartoony label art.

(Note: You must make your NTSC NES universal by cutting the lockout chip to play)

On the back of the cart is a sticker label to personalize your cart with name, address, number, etc. The writing is all in Portuguese. Below the owner's label is the address to Dismac's headquarters:
